This policy explains how the Patient's Choice Primary Care website handles information. It covers this website only. Your medical records and the care you receive at our office are protected under federal health privacy law (HIPAA); ask the office for a copy of our Notice of Privacy Practices.

We do not collect health information on this website

Please do not send health details through this website. Our callback form has no message box on purpose. If you have a medical question, call the office at (540) 307-5495. If you have an emergency, call 911.

The callback form

When you ask us to call you, the form sends us:

• Your name and phone number
• The general reason you chose from the list, such as a new patient or a billing question
• The best time to call

We use this only to call you back. It is delivered to the office by email through a secure mail service run by our website provider, BizPinPro, which also filters out spam. BizPinPro's form privacy notice (opens in new tab) explains that part in detail. We do not sell or share your information for marketing.

Website statistics

We count visits to this site with a privacy-focused analytics system run by our website provider. It does not use cookies or advertising trackers. It records which pages are viewed, roughly where visitors are located (city or region), and how they found the site, so we can tell whether the website is helping people find us.

Booking, maps and outside links

Online appointment booking is handled by Tebra, and the map on our page is provided by Google. When you use them, their own privacy policies apply. Our patient forms are PDF files you print and bring with you; nothing you write on them is sent through this website.
